Routing Directions:

1.    Form is to be filled out by person desiring transportation.
2.    Principal/Supervisor will forward your request, if approved, to Dave Lucas at          DaveLucas@k12.sd.us.  Dave Lucas will reply to all with the vehicle assigned.
3.    If a gas card is needed, please check one out with Dave Lucas.
4.    Pickup vehicle and keys before the bus garage closes at 5:00 pm.
5.    If you need a Substitute make sure you have your paperwork finished.
